  • FIRST, fill in the applicant's general information. NOTE: "CASE DESCRIPTION" can be left blank.

  • SECOND, click the "STATE" dropdown and select the applicant's state of residence.

  • THIRD, click on the "PRODUCT TYPE" dropdown to select the product. For Final Expense (Living Promise) or Children's Whole Life, click "WHOLE LIFE". For IUL's, click "INDEXED UNIVERSAL LIFE". For Terms, click "TERM LIFE".

  • FOURTH, click on "FIND AVAILABLE PRODUCTS"

  • FIFTH, for Final Expense, click on "SELECT" for "LIVING PROMISE"; For IUL's, click on "SELECT" for "Indexed Universal Life EXPRESS"; For Terms, click on "SELECT" for "Term Life EXPRESS

  • FIRST, confirm all of your producer information is correct.

  • SECOND, for question "WILL THERE BE AN ADDITIONAL PRODUCER ASSOCIATED WITH THIS CONTRACT?", If no agent commission split, select "NO". If there will be a commission split between you and another agent on the application, click "YES". For commission splits, both percentages must equal to 100% combined. For example, 75%/25%, 50%/50%. The other agent's last four, birthdate, and email must be entered as well.

  • THIRD, click "NEXT"

  • SECOND, choose how the applicant will receive the signature link. TEXT is preferred if phone# is mobile.

  • THIRD, verify that their email or phone number is correct and that the last four digits of their social is the PIN.

  • FOURTH, click "SEND" and then use the following Signature Guide to walk the applicant through the signature process.

"Now [Applicant's Name], I'm going to send you a [Text / Email] with a link that will let you sign the HIPPA agreement. This just allows the company to run the electronic medical check. So you should be getting a message from a 844 number that says "HELLO [Applicant's Name], YOUR APPLICATION IS READY FOR REVIEW AND SIGNATURE". Go ahead and let me know when you got it so I can walk you through because it can be a little tricky sometimes."​

Ok, go ahead and click on the blue link that starts with "HTTPS". If you get a pop up blocker, go ahead and click "OK" or you can click on where it might say "ALWAYS SHOW" on the top."

“Now on the next screen, you should see a message that says “WELCOME” on top, and then right below that, it should say “YOUR INSURANCE APPLICATION IS AVAILABLE FOR REVIEW”. Below that will be a box where it says to enter the last 4 digits of your social. Go ahead and click inside of that box and enter your last four, then click the blue button below it that says “SIGN IN”.”

“Once you are signed in, you should see where it says “WELCOME, [Client’s Name], TO BEGIN THE HIPPA SIGNATURE, PLEASE REVIEW THE AUTHORIZATION”. Go ahead and scroll down to the yellow box that says “I AGREE AND CONSENT TO APPLYING MY SIGNATURE” and then click the white checkbox.”

“After you click the checkbox, it should now show a box at the bottom of the screen that says “THANK YOU FOR SUBMITTING YOUR ELECTRONIC APPLICATION…YOUR HIPPA SIGNATURE PROCESS IS NOW COMPLETE”.”

Once the applicant signs, you can now click "NEXT". You may have to refresh the page or click on one of the previous tabs on the left for the E-app to recognize the signature is complete in order to move forward.

  • SECONDARY ADDRESSEE. A Secondary Addressee is someone other than the insured / owner that can receive notifications that the policy is in danger of cancelling or lapsing.

  • DRIVER'S LICENSE. A Driver's License is not needed for Final Expense (It helps for identification so obtain DL information if possible) however, not having a license may be a red flag for a Term or IUL application. They will ask additional questions as to why a the client does not have a license.

  • WILL THE PROPOSED INSURED BE THE OWNER? To keep it simple, try to keep the insured and owner the same. However sometimes someone may want coverage on a loved one (Child getting a policy on the parent to cover funeral expenses for example).

  • Click "NEXT".

  • Complete Insurance History Section. IF the client has any recent or current policies with Mutual, it will find it and disclose it here. It will ask if the applicant will allow you to view the information. Once they give you permission, you can click "EDIT" and it will allow you to answer if the current application will replace existing or recent coverage on file. Even after completing existing coverage with Mutual, you will need to reenter the existing or recent coverage below where it asks about coverage details.

  • IF no coverage, select "NO" on both and then click "NEXT".

  • FIRST, enter the coverage amount that the applicant is applying for.

  • SECOND, click "NEXT".

IMPORTANT NOTE FOR IULs: For IUL (Left), on the drop down with the label that says "PERMANENT LIFE", make sure that the selection is "Indexed Universal Life Express WITH EASY SOLVE". This function automatically structures the IUL to last to age 120. However, if you wish to structure it your self, you will need to create an illustration using WINFLEX, and attach it to the application when prompted.

  • PRE-APPROVAL TYPES. At this point, the application will give a pre-approval. For final expense, there are two levels, "LEVEL" and "GRADED". If the applicant is applying for the LEVEL coverage, but at this point shows that they may qualify for GRADED, you will need to let the client know the change in rates and policy specifics.

  • FIRST, IF client shows a Favorable Pre-approval, click "LOCK DATA & CONTINUE APPLICATION".

  • SECOND, click "NEXT".

  • NEXT, complete the health questionnaire and review the answers in the summary.

  • NOTE for IUL/Term applications: Applicant may be denied due to unemployment apart from being a student, or occupation in Trucking, Transportation, Gas / Oil, Law Enforcement and Military. You may choose "Administration" if they happen to work as a supporting role in these types of companies or occupations.

  • Click "CONTINUE"

  • Click "CLICK HERE TO ADD" to add a primary beneficiary. If there are more than one beneficiary, the percentage with all combined must equal 100%. For example, if there are two, then the percentage for both should be something like 50/50 or 60/40. If three, 34/33/33.  Same applies to Contingent. NOTE: A "Contingent" is a back up beneficiary if the primary cannot receive the death benefit. It is not mandatory to add a contingent. A contingent can be added later by the applicant.

  • IMPORTANT: Beneficiary must be 18 years of age and over.

  • FIRST, click "INDIVIDUAL" to keep it simple if dealing with a lead. "Not an individual" means the applicant would like the death benefit to go to a trust, company, church or charity for example.

  • SECOND, fill in the Beneficiary's name, relation and percentage of death benefit. Date of birth is not required.

  • THIRD, click "SAVE".

  • FOURTH, when all beneficiaries have been entered, click on "NEXT".

  • FIRST, confirm all information is correct, then click "CALCULATE MODEL PREMIUM".

  • SECOND, select "MONTHLY BANK SERVICE PLAN".

  • THIRD, for "WOULD YOU LIKE TO PAY YOUR INITIAL MODAL PREMIUM BY CREDIT CARD?" select "NO" as you will still need a bank account to complete the application.

  • CONDITIONAL RECEIPT. This is must be printed and signed by the applicant if used. This is optional and means that as long as there is payment information on file and a promise to pay before the policy start date, the applicant may have coverage under certain conditions.

  • FOURTH, click "NEXT".

  • FIRST, choose who will be the payor. To keep it simple when dealing with leads, try to keep the payor the same as the owner/insured so that you can continue to E-sign and get a instant decision. If the payor is not the owner/insured, the application may need to be printed and signed by all parties.

  • SECOND, select choose "CHECKING" or "SAVINGS" and enter applicant's banking information.

  • THIRD, for "ONGOING AUTOMATIC PAYMENTS" select a day of the month the applicant would like the ongoing payments to be drafted OR you can choose a week and weekday which aligns with the applicant's Social Security Deposit schedule.

  • FOURTH, choose if the applicant wants their initial premium to be drafted upon approval so that coverage can begin on first payment immediately or choose a specific date if they cannot pay the premiums at that time.

  • FOURTH, click "NEXT".

box will pop up that will show a rotating circle icon until the application and disclosures show up. You can read the entire document, or you can press “Close” to proceed.”

“Now go ahead and scroll down to the blue box that says “We have detected that you are signing the application from a touch enabled device”…and below that box, you’ll see a question that says “Would you like to sign the application with your finger?”.
To make things easier, go ahead and
click “NO”, and then you’ll see a blue
button that says “Apply esignature”. Go ahead and click on that blue button to apply signature.”

“If you would like to sign with your finger, go ahead and press “Yes”, then inside the white box with the grey outline, go ahead and draw your signature, it don’t have to be fancy.” Once you’ve signed, go ahead and press the “Capture” button below the signature box.”

“After you sign, it should now show a box at the bottom of the screen that says “Thank you for submitting your electronic application. Your esignature process is now complete”. Do you see that box?”

  • SECOND, answer what your relation to the insured is. You may write a policy on yourself and your family, however you will be paid "ON EARNS" which means that as the monthly payment is received, you will receive the monthly commission.

  • THIRD, select "YES"

  • FOURTH, this must be answered "YES".

  • FIFTH, if in person, select "YES". If done virtually or over the phone, select "NO". In the drop down box that asks how the application was taken, enter "Virtual" or "Phone".

  • Leave additional information or comments blank as this will send the application to underwriting and an instant decision will not be available.

  • SECOND, center the city you are signing in. NOTE: If you are completing this application for an applicant that is outside of your state, you will see their state in the "SIGNED AT STATE" box. This is fine.

  • THIRD, click on "APPLY ESIGNATURE".

  • FOURTH, click on "SUBMIT TO UNITED OF OMAHA".
